Medicare Health Maintenance Organizations (HMOs) are private plans that the federal government pays to administer Medicare benefits. Like all Medicare Advantage Plans, HMOs must provide you with the same benefits, rights, and protections as Original Medicare, but they may do so with different rules, restrictions, and costs.

Some HMOs offer additional benefits, such as vision and hearing care. Eligibility and costs basics You must have both Parts A and B to join a Medicare HMO. Generally you will continue paying your Medicare Part B premium, though some HMOs will pay part of this premium. Some HMOs may charge an additional premium, on top of your Part B premium. If you want Part D coverage, you will receive it through your HMO. Plans may charge a higher premium if you also have drug coverage.

Note: If you join a Medicare Advantage Plan and you want Part D coverage, you must receive coverage from your plan. You cannot enroll in stand-alone Part D coverage unless you join a Medical Savings Account (MSA) or Private Fee-for-Service (PFFS) plan that does not offer prescription drug coverage.

Benefits Access Basics

Once you have joined an HMO, you should receive a benefit card from your plan. You will use your HMO benefit card instead of your Medicare card when you go to the doctor or hospital. In most HMOs, you must see in-network providers to receive coverage, unless you need emergency medical treatment.

Some HMOs offer a point-of-service (POS) option, which allows you to go out of network for certain services. In these cases, you will be covered but usually at a higher cost. Medicare HMOs are not available everywhere. We can Check the availability

  • Is the plan a regular HMO plan or is it a more limited consumer choice health benefit plan? If it’s a consumer choice plan, what services are excluded. Does the plan provide all the coverage I need?
  • A health maintenance organization (HMO) is a type of health plan that provides care to members through a network of doctors, hospitals, and other providers. The providers in an HMO’s network have agreed to treat HMO members at a discounted rate. This allows the HMO to control costs, so out-of-pocket costs are generally lower in an HMO than in other types of health plans.

Denials of services, treatments,  or medications

  • HMOs will pay only for services, treatments, and prescription drugs that are medically necessary. The process they use to service.
  • HMOs must have a process for you to appeal decisions to deny coverage for a treatment or service. You may also appeal.
  • HMO’s decision to deny a prescription drug because it’s not on the approved list.
  • If you lose your appeal, you can ask for an external review by an Independent Review Organization (IRO) to review the denial. RO
  • You can ask for an external review if the HMO decides that the covered service or treatment isn’t medically necessary or is experimental or investigational. You can’t ask for an IRO review if your HMO denied the treatment because the HMO doesn’t cover it.

Customizing your Policy  

You can usually add features or other coverages to your policy so it better suits your needs. You do this by buying policy riders. Some of the most common riders are:

  • Additional term insurance adds term life coverage to a permanent life policy. For instance, if you need $500,000 worth of total coverage, you could buy a $100,000 whole-life policy with a $400,000 term life rider. As you make more money, you could convert the term life rider into a universal life policy or buy an additional whole-life policy.
  • Guaranteed insurability lets you buy additional coverage regardless of your age or health. The company may still use these factors to decide on your premium. You usually must buy the additional coverage by a specified date or life event, such as when you retire or before you turn 50.
  • Accidental death provides an additional payment if you die because of an accident. For instance, if you have a policy with a $500,000 death benefit and a $500,000 accidental death rider, your beneficiary would get $1 million if you die because of an accident. There are some restrictions.
  • Disability waiver of premium covers the premium if you meet the policy’s definition of disabled. This rider is usually only available to people younger than 60.
  • Accelerated death benefit option prepays some or all of the death benefit while you’re still living. You must have a terminal illness, specified disease, or long-term care illness. People often buy this rider to help pay long-term care expenses in case they need them later.
  • Spousal rider provides term life insurance for your spouse. Basically, this rider combines two policies into one.
  • Children’s rider provides term life insurance for your children. Most companies require the child to be at least 14 days old. Coverage typically lasts until the child turns 21 or 25.

  • Final expense insurance, also known as burial insurance, is a type of life insurance that is specifically designed to cover outstanding expenses and funeral costs after a loved one passes away. Unlike pre-paid funeral plans, final expense insurance can be used by the beneficiary you designate as and where needed rather than being limited to specific funeral services and providers. With the cost of an average funeral running into the thousands, a final expense policy can provide much needed protection from not only burial expenses, but also any remaining medical costs, potential legal costs as well as other outlays.
  • Final expense policies will have a lower face value than most traditional term or whole life policies as they are intended for a specific purpose of covering those final costs rather than providing comprehensive support for surviving family members. These type of policies generally don’t require a medical exam, but premiums will be higher the older you are, and some benefit payouts may be limited during the first few years of coverage for those with significant health issues.

How life insurance pays the death benefit

Companies usually pay the death benefit as a single lump sum, but there are other options. Either you or your beneficiary chooses how the death benefit will be paid. Common options include:

•     Interest option. The insurance company keeps the death benefit and pays the interest to your beneficiary at regular intervals.

•     Fixed period. The company pays the death benefit at regular intervals, with interest, over a chosen period

•      Life refund. The insurance company pays a set monthly amount to the beneficiary for the rest of his or her life. Under this option, the beneficiary could get more than the policy’s stated death benefit if he or she lives longer than expected.
